Imaging Case of the Week 198 Answer

The supine abdominal x-ray indicates severely dilated large bowel loops with mucosal oedema (as per the thumb printing sign). The erect abdominal x-ray show multiple air fluid levels. The x-ray is concerning for a toxic mega colon, a well-known complication of ulcerative colitis.

What is the diagnosis?

A 55 year old man had chest and abdominal discomfort after a long haul flight. The chest discomfort resembled his usually stable angina pectoris. On admission his heart rate was 90 beats/min and his blood pressure was 95/52 mm Hg. Electrocardiography showed T wave inversion in the inferolateral leads, compatible with ischaemia, although serum troponin and D-dimer tests were negative. His arterial blood gases, pH, and lactate were normal. What does the supine abdominal radiograph show and what sign suggests the diagnosis (fig 1⇓)?

The abdominal x-ray shows a thumb-printing sign, as seen below, which is indicative of mucosal oedema of the large bowel due to colitis from a variety of conditions (e.g. ulcerative colitis, pseudomembranous colitis, and ischaemic colitis).
