A 24 year old woman is brought to the emergency department semi-conscious after a suspected overdose; empty packs of paracetamol (acetaminophen) and diazepam are found with her. She is also taking carbamazepine for seizures. Her paracetamol concentration at the time of admission is 100 mg/L (660 µmol/L); she probably ingested the pills four to eight hours earlier.

Not a typical case of bilateral knee osteoarthritis

A 64 year old woman presented to our clinic with intermittent bilateral knee pain for several years. She described her pain as mild, diffuse, both improved and worsened with walking, and occasionally associated with a sensation of instability. Although she had no history of trauma, mechanical symptoms such as locking, interference with daily activities, or pain in other joints, she did have osteoarthritis of the knee and osteoporosis. Several years earlier she had received a corticosteroid injection in her left knee but had never undergone imaging of her knees. Recently, she had been using paracetamol (acetaminophen) and naproxen as needed for the pain.

ANd I loved your "important clues hiding in plain sight"So this is another kind of fraud: Reporting on "no differences" found by chance testing non prespecified comparisons.By definition it won't be found in COMPAREWe are monitoring all trials published in the top five medical journals (NEJM, JAMA, The Lancet, Annals of Internal Medicine, BMJ). We are analysing each trial for outcome switching, by comparing the clinical trials registry (or ideally a trial protocol dated before the trial began) against the trial report.
