A pregnant woman with anaemia and thrombocytopenia

A 28 year old woman who was 31 weeks pregnant attended the emergency department of our hospital with acute onset of abdominal pain in her right upper quadrant. She had undergone regular antenatal check-ups in the midwifery clinic, with no problems reported. She had no medical history of note and was not taking any drugs. She was a non-smoker and before she was pregnant she rarely drank alcohol. Her cardiovascular, respiratory, and neurological examinations were unremarkable and she had no peripheral oedema. The fetal heart sounds were normal. Her blood pressure was high (136/104 mm Hg) and her pulse was 88 beats/min. The urine protein to creatinine ratio showed no evidence of proteinuria.

Erythropoietin helps people with kidney failure and symptoms from anaemia who are not yet on dialysis | Cochrane

Anaemia (low red blood cells) is a common complication of kidney failure. Anaemia causes some of the tiredness and problems associated with kidney failure. Manufactured erythropoietin (a hormone that increases red blood cell production) improves this, and is used by people on dialysis (treatment from an artificial kidney machine). This review found it can also reduce anaemia for people with kidney failure who are not yet on dialysis. It is not known if erythropoietin use can delay the need for dialysis.

Autoimmune haemolytic anaemia: rituximab | key-points-from-the-evidence | Advice | NICE

Limited high‑quality evidence was identified that investigated how well rituximab works for treating autoimmune haemolytic anaemia. One randomised controlled trial suggested that after 12 months, prednisolone plus rituximab was more effective than prednisolone monotherapy for inducing a complete response to treatment in adults with newly diagnosed and previously untreated warm autoimmune haemolytic anaemia. Other uncontrolled studies suggested some effectiveness of rituximab in warm and cold autoimmune haemolytic anaemia, but limitations of these studies make it difficult to draw any firm conclusions.

Intermittent regimens of iron supplementation during pregnancy | Cochrane

Anaemia is a frequent condition during pregnancy, particularly among women from low- and middle-income countries who have insufficient iron intake to meet increased iron needs. Traditionally, pregnancy anaemia has been prevented with the provision of daily iron supplements, however, it has recently been proposed that if women take supplements less often, such as once or twice weekly rather than daily, this might reduce side effects and increase acceptance and adherence to supplementation. In this review we assess the benefits and harms of intermittent (i.e. two or three times a week on non-consecutive days) oral supplementation with iron or iron + folic acid or iron + vitamins and minerals for pregnant women.
