Endometriosis is linked to greater risk of complications in pregnancy and birth, study finds

Women with endometriosis have a higher risk of miscarriage and ectopic pregnancy, a large cohort study in Scotland has found. Women with the condition who maintain a pregnancy for more than 24 weeks face further risks, including a higher risk of placenta praevia, bleeding before and after birth, and caesarean section.

Risk of intracranial haemorrhage linked to co-treatment with antidepressants and NSAIDs

The linked paper by Shin and colleagues (doi:10.1136/bmj.h3517) offers important new evidence about the relation between antidepressant drugs, non-steroidal anti-inflammatory drugs (NSAIDs), and the incidence of intracranial haemorrhage.1 Although NSAIDs and antidepressants are both associated with an increased risk of gastrointestinal bleeding, previous evidence has not shown clear links between intracranial haemorrhage and either type of drug when used alone. The new study, however, set out to define the risk of such bleeds when the two classes of drugs are combined, and the results give some cause for concern. In a retrospective population cohort study of more than four million people in Korea, the authors found that the combination of antidepressants and NSAIDs was associated with a substantially increased risk of intracranial haemorrhage, compared with antidepressant treatment alone.

Retrospective cohort studies: advantages and disadvantages

Researchers investigated whether differences exist between the sexes in the risk of ischaemic stroke in patients with atrial fibrillation.1 A nationwide retrospective cohort study design was used. Data were taken from the Swedish national discharge register. Participants were 100 802 patients with a first diagnosis of atrial fibrillation between 1 July 2005 and 31 December 2008, with a total follow-up of 139 504 years at risk (median 1.2). Information about drug treatment was taken from the Swedish drug register. Patients were excluded if at baseline they were prescribed warfarin, had mitral stenosis, or had previous valvular surgery. Patients who died less than 14 days from baseline were also excluded.

In a cohort study (2005–2011) including all patients with first-time ICU admissions in Denmark (n=57,110), we compared patients with and without pre-existing AF and estimated absolute risks and relative risks (RRs) of arterial thromboembolism and death within 30 days and 365 days following admission, using Kaplan-Meier methods and multivariate regression analyses. We analysed the prognostic impact of AF within strata of patient age, sex, coexisting cardiac diseases, and ICU therapies.
