Eczema (Dermatitis)

Eczema (aka Dermatitis) presents as a poorly demarcated, itchy rash. There are several causes. Henoch Schonlein Purpura (HSP)

This is a vasculitis that most commonly occurs in children. It tends to only affect the small vessels, and typically presents with: Palpable purpura – red/purple discolorations in the skin, often on the extensor surfaces of the feet, legs, arms, or sometimes on the buttocks. The rash may initially resemble urtricaria, but later becomes palpable. GI disturbance – may include colicky abdominal pain, abdominal tenderness, melena – occurs in 50% of patients

A 28 year old woman presented with a raised, red and intensely itchy rash on the extensor surface of her elbows and buttocks, in groups of small blisters. There was no history of trauma, atopy, or recent illness. She had no gastrointestinal symptoms or change in bowel habit and had not lost weight. No major medical or surgical history was noted and she was not taking drugs on a regular basis. Owing to the appearance and location of the rash, together with pruritus, dermatitis herpetiformis was suspected.

A rash in a patient with neutropenia

A 72 year old man with relapsed acute myeloid leukaemia developed a rash seven days after starting a combined chemotherapy regimen (fludarabine, cytarabine, granulocyte colony stimulating factor, and idarubicin). He first noticed a painful lesion on his left forefoot, and over the next seven days he developed similar lesions on the left lower limb (fig 1⇓), right thigh, and face. He was referred for a dermatological opinion on day 14.

A 42 year old man develops a sore throat. He does not consult his GP but takes some over the counter painkillers and an anti-inflammatory. 1 week later he has a non itchy rash starting on his feet then spreading to his trunk. The rash has the appearance of tiny teardrops. He is correctly diagnosed with guttate psoriasis. The patient is put on penicillin orally 250mg QDS for 1 week. He then starts passing small volumes of urine. His urine is described as "orangey brown" . He continues taking a non steroidal anti inflammatory drug (ibuprofen 800mg TDS).
