An unexpected finding after a fall from a horse

A 37 year old jockey was admitted with left sided abdominal pain and fullness one week after a fall from a horse. Before this he had been fit and well. On examination, he was tender in the left upper outer quadrant of his abdomen and his blood pressure was 90/55 mm Hg. A computed tomography scan showed an area of active bleeding in the parenchyma of the spleen and a large subcapsular haematoma.

An abnormality at the hepatic flexure

A 92 year old woman presented to the emergency department after collapsing at home. She recalled standing from her chair, feeling lightheaded, and then collapsing. She had felt generally weak for more than a year, with weight loss of 56 lb (25.2 kg) but no change in bowel habit, dysphagia, or gastrointestinal bleeding. Her medical history included hypertension, hypothyroidism, and anaemia (which was currently being investigated by her general practitioner). Among other drugs, she was taking lisinopril, bendroflumethiazide, and levothyroxine. Her son had died at 60 years of age from large bowel obstruction and perforation secondary to colon cancer.

There is this claim that by consuming mangosteen with something that is sweet (sugar), one would die, due to haemorrhage and so on. Is there any merit to this?

Why are Sengstaken–Blakemore tubes rarely used?

I have read that Sengstaken–Blakemore tubes are used for a bleeding oesophageal varices. However, I was talking to a doctor in hospital the other day and they said that they are not often used in actual practice. Is this true and if so why?
