Amid Class 2 Watchman Recall, CMS Proposes Limits on Coverage

While the CMS proposes reimbursement only for afib patients not on warfarin, the FDA warns of blood-leakage risks if the device's hemostasis valve is "tightened with the dilator in place."

weve-got-a-pumper-here

Hemostasis is an essential step in wound management. Most commonly, bleeding is caused by lacerated subdermal plexus and superficial veins which can be controlled with pressure alone. When lacerations are especially deep, an artery may also be affected. In these situations, special maneuvers are often necessary to obtain adequate hemostasis.

Prothrombin complex concentrate for reversal of vitamin K antagonist treatment in bleeding and non-bleeding patients | Cochrane

Prothrombin complex concentrate (PCC) is a drug that contains a source of proteins involved in the human blood clotting process. Patients medicated with vitamin K antagonists (blood thinning drug) have low blood levels of these important blood clotting proteins. Therefore these patients will be at increased risk of spontaneous and traumatic bleeding events. Also, when these patients experience a bleeding event, this will lead to progressive loss of these important blood clotting proteins. This process causes a vicious circle, thereby increasing risks of illness and death.

Snake antivenoms for treating people who have been bitten by a snake, and have developed abnormal blood clotting | Cochrane

Many snake venoms cause coagulopathy in humans. Coagulopathy is a condition in which the person’s blood is unable to clot because the venom causes decreased levels of clotting factors. Coagulopathy increases the risk of bleeding. Antivenom is a treatment used to neutralise venom in people who have been bitten by a snake. There is some evidence from observational studies in humans which suggest that snake antivenom is helpful to people who have been bitten by a snake. However, the use of antivenom has some risks, and can cause allergic reactions.

Critical Care

Patients with severe sepsis often present with concurrent coagulopathy, microcirculatory failure and evidence of vascular endothelial activation and damage. Given the critical role of the endothelium in balancing hemostasis, we investigated single-point associations between whole blood coagulopathy by thrombelastography (TEG) and plasma/serum markers of endothelial activation and damage in patients with severe sepsis.
