Wilson's Disease

Inheritance: autosomal recessive Pathology:copper incorporation into caeruloplasmin in hepatocytes and and its excretion into bile is impaired so copper is deposited into organs (firstly liver, then basal ganglia) Clinical: - Presents in children with liver failure/jaundice - Presents in young adults with CNS signs e.g. ataxia, tremor, dysarthria  - Eyes: kayser fleischer rings (gold ring around iris)

Autosomal dominant polycystic kidney disease (ADPKD)

ADPKD is an autosomal dominant disorder characterised by the presence of cysts in both kidneys. The cysts are numerous and fluid-filled, resulting in massive enlargement of the kidneys, often with progression to renal failure. The disease can also affect the liver, pancreas, and rarely, the heart and brain.  ADPKD has an incidence of 1/1000 and account for approximately 5% of patients with ESRF requiring replacement therapy.

Physiological Changes of the Heart at Birth

Fetal circulation The fetus receives blood via the umbilical vein. Once this enters the body, it travels through the liver as the ductus venosus before joining the inferior vena cava. It allows 80% of fetal blood to bypass the liver on return from the placenta. It closes functionally within minutes of birth, and structurally within a couple of weeks (may take longer in pre-term babies) Becomes the ligamentus venosum

LFTs - Liver Function Tests

Almost every patient admitted to hospital will have their liver function tested, along with a full blood count, urea and electrolytes, glucose, and probably CRP. A typical set of LFTS will include: Bilirubin - can show pre-hepatic, intra-hepatic and post-hepatic causes of jaundice. A patient won't necessarily be visibly jaundiced if bilirubin is raised, especially if the bilirubin is not grossly elevated.
