Potassium plays a major role in regulating the skeletal muscle blood flow; an increased potassium concentration in the muscle during muscle activity causes vasodilatation, which increases the regional blood flow. In state of hypokalemia, this increase is hampered causing relative ischemia in the active muscle consecutively leading to muscle cramps, and in a severely depleted state may cause muscle necrosis and rhabdomyolysis. In addition to hypoperfusion, hypokalemia-induced impairment in muscle metabolism also may contribute to muscle dysfunction.

Branched-chain amino acids improve symptoms of hepatic encephalopathy | Cochrane

Hepatic encephalopathy is a brain dysfunction associated with liver disease. Cirrhosis, which is a condition where scar tissue (fibrosis) replaces the normal liver tissue, is the most common cause of hepatic encephalopathy. The severity of the symptoms range from minor signs to coma. The minor changes are known as minimal hepatic encephalopathy. Overt hepatic encephalopathy refers to the more severe stages with clinically apparent manifestations such as changes in the level of consciousness or neuropsychiatric abnormalities. Many people with cirrhosis lack amino acids, which are building blocks of proteins. The amino acids with a side-chain (a branch) are known as branched-chain amino acids (BCAA). The BCAA play an important part of the generation muscles and of the signalling chemicals in the brain. These effects may benefit people with hepatic encephalopathy.

Pentoxifylline for intermittent claudication | Cochrane

Atherosclerosis, or hardening of the arteries, results in narrowing and blockage of the arteries and can reduce the blood supply to the legs, causing peripheral arterial disease. Intermittent claudication (IC) is a cramp-like pain felt in the leg muscles that is brought on by walking and is relieved by standing still or resting. Pentoxifylline is a drug that is used to relieve IC while improving people's walking capacity. It decreases blood viscosity and improves red blood cell flexibility, promoting microcirculatory blood flow and increasing oxygen in the tissues. This review looked at all available evidence from randomised controlled trials on the efficiency of pentoxifylline for treatment of IC.

Breathing training before surgery for reducing lung complications after surgery in adults undergoing heart and major abdominal surgery | Cochrane

Despite advances regarding patient care in the last few decades, breathing complications as a result of lung injury after surgery such as pneumonia are the leading cause of sickness and death in adults undergoing heart and major abdominal surgery. Training of breathing muscles using a small device at home before surgery seems to make breathing easier and helps strengthen muscles of respiration after surgery. This training may help reduce breathing complications after surgery and may lead to improved patient care and overall health care cost savings for the public health system. We wanted to establish whether training of breathing muscles before surgery can reduce the risk of lung complications and to identify who in particular might benefit from such training.

Radiofrequency denervation for chronic low back pain | Cochrane

Background Low back pain is a widespread problem that has major social and economic consequences. In all, 85% to 90% of low back pain cases are classified as 'non-specific'. Most patients with low back pain are treated successfully in primary care, but approximately 10% to 15% develop chronic symptoms (lasting longer than three months). Chronic low back pain can come from any part of the back that has a nerve supply capable of transmitting pain signals. These sources include discs, vertebrae, sacroiliac joints, facet joints, muscles, ligaments and other structures. Pain specialists try to identify the source of low back pain by using nerve blocks. They numb individual spinal nerves with anaesthetic injections to see if this leads to improvement in back symptoms. With substantial pain relief, they attempt to eliminate pain for a longer time by heating the spinal nerves with radiofrequency waves to ensure that the pain stimulus cannot be passed. This invasive procedure is called radiofrequency denervation. At this time, the effectiveness of this approach has not been proven.
