Alternatives to statins in intolerant patients do lower cholesterol, study finds

Statins do cause painful muscle cramps in some patients, and alternative drugs can reduce low density lipoprotein cholesterol (LDL-C) levels, a two stage randomised clinical trial published online in JAMA has shown.1

PUSH HARD AND FAST – AND SAY NO TO DRUGS

Drugs in cardiac arrest has been under bombardement for some time now. Atropine is out, adrenaline is just in there for good old times sake. And now, in an unselected cardiac arrest population, a new NEJM published RCT shows that the silver bullet anti-arrhythmic amiodarone is no more effective in getting better outcomes than injecting saline. The same went for the old Hail Mary rescue drug lidocaine, also included in the RCT. The basics are what matters in cardiac arrest: early and effective chest compressions and early defib.

Novartis is fighting a demand from US attorneys for thousands of documents detailing 80 000 events in which doctors allegedly received money and expensive meals as kickbacks from 2002 until at least January 2011 in return for prescribing the company’s drugs and billing various public payers. The government claimed that thousands of meetings that Novartis described as speaker programmes were really just paid entertainment for physicians. It is seeking as much as $3.4bn (£2.4bn; €3bn) in damages.

A man with a short history of lower back pain

A previously fit and well 69 year old man presented to his general practitioner with a four week history of lower back pain. He had no history of trauma, took no regular drugs, was an ex-smoker, and did not drink alcohol. Physiotherapy, sought privately, had provided no benefit. His GP prescribed painkillers and reviewed him two weeks later, at which point, with no improvement in symptoms, spinal radiography was organised. Radiography showed multiple compression fractures of vertebrae T11-L2 (fig 1⇓; arrows).

ALPS: No Overall Survival Increase From Amiodarone, Lidocaine

Although no benefit was found in the full group assessed, subgroup analysis showed significantly improved survival rates for both drugs vs placebo in those with witnessed out-of-hospital cardiac arrest.
