Maternal preconception and gender selection

Maternal preconception and gender selection has long been a controversial topic. Are you more likely to conceive a boy if you eat red meat, and a girl if you make love under a full moon? Oldwives tales and fantasies exploring sexual position, diet, and dominance circulate the Internet however how can we logically distinguish between fact and fiction? The topic has widespread cultural implications. Sex-related abortions are on the increase in China and India where local customs and religious virtues appear to strongly correlate with the systematic elimination of girls. In an effort to challenge the dogma of chance fertilization two main research streams have explored variations in maternal condition and gender conception. The ‘Maternal Dominance’ hypothesis has suggested trait dominance, underpinned by serum testosterone, correlates with increased male conception rates. The second, ‘Maternal condition’ hypothesis relates to pre-conceptual maternal diet, investigating variations in both quantity and quality of diet and effects on sex ratios. However such assumptions have been difficult to replicate and more recent evidence has suggested changes in maternal condition may have a stronger influence Maternal adaptations in behavior appear to closely correlate with biased gender ratios and can have wider connotations on sex-linked disease inheritance. However unless we can identify molecular mechanisms influencing the intrauterine environment and follicular development, hypothesis will remain mere assumptions.

Legal and Ethical Issues in Medical Treatment of Children with Variations of Sex Anatomy

Speaker: Anne Tamar-Mattis An estimated 1 in 2,000 babies is born with a Difference of Sex Development (DSD or intersex condition). Currently, there is much controversy regarding the best course of treatment for those children with DSD born with atypical genitals. However, little attention has gone to the process of decision-making, or to other important questions such as sterilization of children with DSD or protecting privacy rights. This workshop begins with an overview of the biology of DSD and the basic legal and ethical principles of surrogate consent in pediatric cases. We will then discuss some key legal and ethical questions. Participants will become familiar with key legal and ethical issues in the treatment of children with DSDs, with special attention to unsettled questions of law and uncertain medical outcomes. Participants will deepen their understanding of the ethics and law of surrogate decision-making for children. Participants will be able to identify potential situations in the treatment of children with DSDs where additional legal or ethical consultation may be indicated.

Weight loss in an adolescent girl

A 15 year old girl who had been previously fit and well presented to the child and adolescent mental health services (CAMHS) team with weight loss, irregular menstrual periods, excessive exercise, restriction of energy intake during the day, and binging during the evening, followed by vomiting. Her symptoms had been present for a year, since her father had experienced a myocardial infarction. Clinically her symptoms were in keeping with anorexia nervosa, as shown by her completed eating disorder questionnaire.1 Her body mass index (BMI) was 16 and her weight for height was 85% of the median (50th centile) for age and sex. She had a distorted body image, believing her stomach and ankles were too fat, and she had an extreme fear of gaining weight.
