On any given Sunday... | Anaesthesia Case Study

You are on call for CCU and you accept a patient for transfer: 56M with recent crescendo angina. He was seen in a peripheral hospital where cardiology performed a diagnostic catheterization but did not perform angioplasty. He has a >90% lesion in RCA. He was loaded with aspirin and ticagrelor and transferred with advanced paramedics in stable condition.

Critical Care

Patients receiving circulatory support with peripheral veno-arterial extracorporeal membrane oxygenation (VA-ECMO) are at risk of developing differential hypoxia. This phenomenon occurs in patients with concomitant respiratory failure. Poorly oxygenated blood, ejected into the ascending aorta from the left ventricle, competes with retrograde flow from the ECMO circuit, potentially causing myocardial and cerebral ischaemia. In a recent Critical Care article, Hou et al. use an animal model of peripheral VA-ECMO to study the physiology of differential hypoxia. Their findings support a dual circuit hypothesis, and show how different cannulation strategies can disrupt the two circuits. In particular, strategies that increase venous oxygen saturations in the pulmonary artery can have a large effect on oxygenation saturation in the ascending aorta. The authors provide evidence supporting the use of veno-arterial-venous ECMO in patients who require peripheral VA-ECMO but have simultaneous respiratory failure.

According to the American College of Cardiology(ACC), based on guidelines published this month, there is a shift in the post cardiac arrest care algorithm.  The prior guidelines from 2013 have a Class I recommendation for performing immediate heart catheterization for PCI for the management of comatose patients with STEMI after out of hospital cardiac arrest.  Prior to now there were no recommendations for post arrest patients who did not have STEMI.  The ACC guidelines now push for immediate catheterization for patient’s who do not have unfavorable resuscitation features (discussed in the table provided) after discussion with our cardiology colleagues.  This may drastically change post-arrest care as the majority of post-arrest patients do not have STEMI on EKG.  The data supporting catheterizing non-stemi post-arrest patients is more limited, but data suggests that about 25% of patient’s without STE will have an occlusion upon cath and about 60% will have significant obstructive lesions.  One study evaluating early invasive treatment in the management of non-stemi post-arrest patients found that there was better long term survival  (60% vs 40.4% [p = 0.005]) and a more favorable neurological outcome (60.0% vs 39.7% [p = 0.004]) when implementing the invasive strategy.

Vascular access in ill neonates who present to the ED can be challenging. Umbilical vein catheterization can be a life-saving option. The umbilical vein remains patent and viable for cannulation until approximately 1 week after birth.

Cardiac catheterisation is the process by which you gain catheter access to the veins or arteries of the heart. Thus, in the procedures of coronary angiography, and angioplasty, you perform cardiac catheterisation as part of the procedure.
