Clinical trials: units of randomisation

Researchers evaluated the efficacy of a short course of oral prednisolone for acute asthma in children of school age. A randomised double blind placebo controlled crossover trial study design was used. The intervention was parent initiated treatment with the corticosteroid prednisolone (1 mg/kg/day) or placebo when an episode of acute asthma occurred. For each episode, treatment with prednisolone or placebo was chosen at random. Participants were 230 children aged 5-12 years with a history of recurrent episodes of acute asthma. Each child could contribute a maximum of eight episodes of acute asthma to the data collection over a three year study period.1

Time to administration of epinephrine and outcome after in-hospital cardiac arrest with non-shockable rhythms: retrospective analysis of large in-hospital data registry

Objective To determine if earlier administration of epinephrine (adrenaline) in patients with non-shockable cardiac arrest rhythms is associated with increased return of spontaneous circulation, survival, and neurologically intact survival.

Screening for lung cancer using low dose computed tomography

Screening for lung cancer with low dose computed tomography can reduce mortality from the disease by 20% in high risk smokers. This review covers the state of the art knowledge on several aspects of implementing a screening program. The most important are to identify people who are at high enough risk to warrant screening and the appropriate management of lung nodules found at screening. An accurate risk prediction model is more efficient than age and pack years of smoking alone at identifying those who will develop lung cancer and die from the disease. Algorithms are available for assessing people who screen positive to determine who needs additional imaging or invasive investigations. Concerns about low dose computed tomography screening include false positive results, overdiagnosis, radiation exposure, and costs. Further work is needed to define the frequency and duration of screening and to refine risk prediction models so that they can be used to assess the risk of lung cancer in special populations. Another important area is the use of computer vision software tools to facilitate high throughput interpretation of low dose computed tomography images so that costs can be reduced and the consistency of scan interpretation can be improved. Sufficient data are available to support the implementation of screening programs at the population level in stages that can be expanded when found to perform well to improve the outcome of patients with lung cancer.

Culture and leadership in the NHS

The King's Fund's second leadership survey revealed a mixed picture of leadership, culture and the working environment across the NHS. In February and March 2014 The King’s Fund conducted a survey of NHS managers and clinicians about leadership, culture and compassionate care in the NHS. We received more than 2,000 responses. This is the second leadership survey of NHS staff we have undertaken and it revealed a mixed picture of leadership, culture and the working environment across the NHS.

Checklist For a Marketing Programs Manager - Marketing Strategies That Drive Go-to-Market Plans - Four Quadrant

Anywhere from 1-10% of a company’s revenues may be allocated to the marketing budget and in most cases the people to programs ranges from 70:30 to 30:70.  A key role in Marketing is the Marketing Programs Manager who can effectively manage about $250-$350K but most companies don’t have a checklist for a Marketing Programs Manager.  The role is so critical that I wrote a document, What I would do if I was a Marketing Programs Manager, and sent it to the Director of Demand Creation and asked that his team paste this in their cubes as a daily reminder of what needs to be done each and every day to be successful in the role.  Here it is.
