This is a podcast on the Clinical Pharmacology that is relevant to the treatment of cardiac failure. In this podcast we shall be covering the therapeutics of: * Acute/Decompensated cardiac failure * Chronic cardiac failure Once again we shall be using this as a means to discuss certain drugs, here ACE-inhibitors, ARBs, beta-blockers, spironolactone and digoxin.

Digoxin is a cardiac glycoside, which have two main effects: Increases the force of contractions (through increased intracellular Ca2+) Reduce conduction through the AV node It is most commonly used to control AF and atrial flutter, and in some cases, digoxin may also be used in heart failure. Digoxin is associated with increased mortality in patients treated for atrial fibrillation or heart failure, the largest meta-analysis to review this treatment has concluded. The researchers warned that digoxin should be used with great caution and that patients’ plasma levels should be monitored until randomised controlled trials are carried out.

However, "if we cannot get a clinical trial to show any significant effect on both safety and efficacy, then I think it is a drug of the past," says the lead author.

Newer drugs have replace digoxin as first line treatment for atrial fibrillation or heart failure, and observational studies have questioned its safety. Is it still a safe and effective choice?

The drop seems steepest in patients seen as susceptible to digoxin toxicity, say researchers, and there's evidence that it's used more "in patient populations that may potentially benefit the most."

This case is written by Dr. Kyla Caners. She is a PGY5 Emergency Medicine resident at McMaster University and is also one of the Editors-in-Chief here at EMSimCases. Why it Matters Digoxin toxicity is of critical importance to recognize. There are many subtleties to its management, which means that the concepts of digoxin toxicity are important…
