Dermatitis herpetiformis

A 28 year old woman presented with a raised, red and intensely itchy rash on the extensor surface of her elbows and buttocks, in groups of small blisters. There was no history of trauma, atopy, or recent illness. She had no gastrointestinal symptoms or change in bowel habit and had not lost weight. No major medical or surgical history was noted and she was not taking drugs on a regular basis. Owing to the appearance and location of the rash, together with pruritus, dermatitis herpetiformis was suspected.

A 60 year old woman, originally from Greece, was admitted after burning herself while trying to warm her legs on an electric heater. She had rheumatoid arthritis treated with methotrexate and adalimumab.

Hoarseness in a 79 year old woman

A 79 year old woman was referred to our ear, nose, and throat outpatient clinic with a history of hoarse voice. This symptom had been present for around three months. It initially fluctuated in severity but eventually became constant. There was no associated pain, weight loss, cough, dysphagia, odynophagia, or other upper airway symptoms. She had not recently had surgery or experienced trauma. Her medical history was of chronic obstructive pulmonary disease, and she was an ex-smoker.

A woman with rapidly progressive weakness and sensory loss

A 61 year old woman presented to the accident and emergency department with progressive weakness of the lower limbs and a feeling of her legs being “frozen” over the preceding four days. Ten days earlier, she had contracted a flu-like illness and was diagnosed and treated with oseltamivir (Tamiflu) for presumed swine flu. She was later found to be influenza A/H1N1 seropositive on the reference laboratory haemagglutination assay. Her medical history included cervical spondylosis for which she had decompressive laminectomy (C4-7) in 1992 and a Cloward’s discectomy with bone graft and fusion of C5-7 in 1996.

A 76 year old woman reports recurrent urinary frequency, dysuria, and malodorous urine. No bacterial growth has been identified on two midstream urine samples, though empirical treatment with antibiotics has improved her symptoms. After three months, an episode of visible haematuria prompts referral and a transitional cell carcinoma of the bladder is diagnosed.

Triptans for symptomatic treatment of migraine headache

A 30 year old woman has debilitating, pulsating right sided headaches up to twice a month for several years. Her pain builds up rapidly and usually lasts 24 hours. She has to lie down during episodes, which are associated with sensitivity to light, severe nausea, and occasional vomiting. Over the counter analgesics such as ibuprofen or paracetamol were initially helpful in relieving pain, but not recently. She asks if any treatments may help with her headaches and allow her to return to her usual activities sooner.
